Prader-Willi Case Essay This essay will discuss the role of the nurse in the context of Prader-Willi syndrome (PWS) which is caused by a genetic disease by a deletion in chromosome 15. This can lead to insatiable hunger, excessive eating and result in obesity. This syndrome requires management from the multidisciplinary team which includes dieticians, doctors, mental health team, nurses, occupational therapist, physiotherapists and social services. This is where the role of the nurse and nursing staff can stand out as they are actively involved in patient care providing support to the patient and their family, as well as playing a role in preventing disease progression. â€Å"Make the care of people your first concern, treating them as individuals and respecting their dignity† (NMC, 2010). This is of utmost importance in the management of PWS, and how the individual can be educated by managing the syndrome. This essay will relate across the lifespan; childhood, adolescence and adulthood. This will be discussed in the following paragraphs. PWS is an uncommon genetic disorder that is present at birth in either male or female. It is the most common genetic cause of morbid obesity and can vary at different weights. Although the cause is complex, it results from a deletion or unexpression of genes from the paternal chromosome 15. This condition affects approximately 1 in 10,000 to 1 in 25,000 new-borns (Killeen, 2004). Individuals with this condition have serious problems controlling their weight as they have a very strong food compulsion before the age of six. The condition is diagnosed through genetic testing. It is specifically DNA-based methylation testing to distinguish the absence of the paternal chromosome; chromosome 15.This test is recommended for new borns with pronounced hypotonia (praderwillisyndrome, 2010). An early diagnosis allows for early intervention as well as early provision of growth hormone (GH) treatment. GH gives an increased muscle mass and supports linear growth. GH treatment also advantageous because it decreases food preoccupation and weight gain. During pregnancy, there can be a few abnormal signs which can indicate, but are not limited to PWS. In utero, there can be excessive amniotic fluid; a condition known as polyhydraminos. There can also be reduced fetal movements and the fetal position within the uterus may be suboptimal i.e. breech presentation. Once the baby is born, other signs such as feeding difficulties- due to poor muscular tone affecting the sucking reflex and generalised hypotonia-poor muscular tone (FPWR, 2011).The baby may feel floppy when held as their joints may be loosely extended instead of being firmly in position. An early diagnosis of these can point to an early diagnosis of PWS, hence lead to early management. The clinical presentation of PWS is not limited to physical signs and symptoms but includes linear growth and development, which can cause mental and behavioural problems. These can be presented early in childhood. Physical features can include short stature, small hands and feet, low birth weight, and classic facial features including narrow forehead, almond-shaped eyes and â€Å"down-turned† mouth (Holm et al, 1993). Behavioural symptoms can include obsessive behaviours, unpredictable temper tantrums, skin picking, stubbornness and resistance to change. Individuals with this condition are not mentally stable as they have an increased risk and suffer from depression and psychosis. They also suffer from hallucinations, loss of interests, changes in mood and poor concentration levels. As mentioned earlier, Hypotonia is poor muscle tone. Hypotonia improves with age, however if it persists by the age of two to three, it is very likely that the child may not have started walking. Walking is a crucial milestone that should be reached within the first two years of life (NLM 2010).This is because their weight gain has made it difficult to move around and their condition is already exacerbated by the hypotonia. They can be referred to physiotherapy to try and improve the muscle tone. They also have a failure to thrive and their rate of physical growth is less than their peers’. With failure to thrive, these infants may not respond to simulation as they tire easily. Infants with this condition gain weight more slowly and start to put on more weight by the age of 2-3. A child with PWS may start speaking later than other children as their verbal skills are delayed. Speech and language therapy is advisable at this point as the child will benefit with input from a ther apist. Most common speech concerns include problems with voice quality, articulation, usage as well as resonance patterns (Munson-Davis, 1988). The child constantly craves for food and eats more than they should. They constantly gain weight and may eat things most people wouldn’t deem edible; such as expired or frozen food. There is a serious compulsion towards food, and a lack of awareness of hunger satiation. In childhood, they have a tendency to be stubborn, argumentative and possessive (Nordqvist, 2010). Some infants can develop obsessive compulsive disorder (OCD) along with repetitive behaviours. They can throw tantrums as they can only consume a certain amount of food per day so they do not gain weight (as per their dietary management plan). During adolescence, height becomes more noticeable as the individual is much shorter than others. The height of a female with PWS on average is 4 feet 10 inches while that a male with PWS is 5 feet 2 inches (nhs.uk, 2011). The individual would still suffer from hypotonia up until adulthood and would be extremely flexible due to poor muscle tone. Once the individual has reached adulthood, they cannot reproduce as they are infertile due to delayed puberty in both male and female from a young age. The reproductive system would not have produced enough sex hormones, which results in undeveloped sex organs. Hypogonadism is a medical term for the reduction or absence of hormone secretion or other physiological activity of the gonards. Individuals with PWS have some degree of a learning disability. Learning disability nursing practice reflected current philosophies of supporting people with learning disabilities (Clifton et al. 1992). The presence of PWS in a family can create substantial stress. Families would have had to adapt to changes within the household to be able to manage the individual with the syndrome. Parents are often exhausted from the demands of their time and energy for diet control, specialized programmes, therapy appointments and behavioural supervision. Siblings are also affected as they often feel neglected as the PWS sibling receives more attention and appears to be more loved. (Tomase-ski-Heinemann 1998) It is a nurse’s responsibility to help support and manage a patient with the condition along with supporting the individual and their family. The uncontrollable appetite leads to obesity. Obesity is a global epidemic, and is also known to be a significant risk factor for other health related problems which include heart disease, diabetes, high cholesterol, hypertension, hypoventilation and right sided heart failure (WHO, 1948). Some people with PWS also develop type 2 diabetes mellitus which is the most common form of diabetes, where the body either does not produce enough insulin or the insulin is not working properly; insulin resistance. In addition, part of the due diligence of nurses is that in providing care for a patient with PWS that they holistically look after the patient starting from the first interaction. For example, in PWS this involves first building a rapport with the patient explaining their diagnosis and describing the nurses’ role in the management (monitoring weight, supporting diet). It should be stressed to the patient and their family the importance of confidentially and that their information will not be told to anyone outside the medical team. This ensures that the nurse has fulfilled their responsibly within the multi-disciplinary team. The Data Protection Act (1998) was put in place to maintain patient records and information. Therefore the nurse would be upholding these legislations by practising patient confidentiality. When visiting a patient at home or in the hospital, a nurse should ask for consent for patient contact i.e. assessing vital signs. Nurses’ must follow the NMC guidelines although the patient may not understand what the nurse is saying because of a learning disability for example. However, consent must be indicated in some form such as nodding of the head. The NMC (2010) states that â€Å"you should ensure that you gain their consent before you begin to provide care†. If the patient is unable to give consent and is alert, the next of kin is assigned to making the decision due to the best interest of the patient. A nurse is accountable to manage, maintain and monitor the individuals’ weight. The nurse does not only have to keep track but also the family should be involved in managing the weight. Nurses can book weekly appointments with the individual and their family/carer so their weight can be monitored to check for any improvement to the weight or not. The family should monitor the amount the individual consumes daily. They are constantly hungry and cry for more food if it is not given to them. Locks must be placed on cupboards or on the kitchen door to stop them from eating (PWSAUSA, 2009). It will be hard for the individual to cope once this is introduced as they do not know when to stop. This is where the nurse should explain to the individual how important it is to manage their weight and what it can lead to if it is not controlled. The nurse should be there to support them when the individual starts to show aggressive behaviour as it will be hard for the family as well. Adults with PWS are inactive due to their low muscle tone and therefore only require 1,000-1,200 calories a day (PWSA, 2010). Encouraging the individual to be healthy is important. Although the nurse must understand that the patient may be unable to exercise properly due to poor muscle tone, they should encourage the patient to eat healthily for example fruit and vegetables. The individual must not have too many fatty foods i.e sweets and chocolate. By promoting healthy foods will ensure that the individual does not gain more weight than they should. It will be hard for the individual to cope with the new foods introduced to them which is why a nurse will be there to support the individual and family. The nurse can also advise the family on encouraging the patient to do some exercises i.e. helping with house chores. Any sorts of movement can help burn calories. Communication skills is one of the key skills a nurse should have. â€Å"To understand the process of communication, we must understand how people relate to each other† (Faulkner, 1982). Supporting and helping patients and their families, communication is crucial. By managing the individual and their condition, team work is fundamental. It is important to work as team as the main focus in the patient care plan is the individual. Each health care professional has a role to play to help improve the individuals well-being. The main focus is the role of the nurse and how their professional issues can impact on the health and illness of people across the life span. It is important for a nurse to understand individuals and their condition because they can help make it somewhat easier for the patient and their family. This is because the nurse is an allied health professional who enjoys more interaction with the patient than many other members of the multidisciplinary team. Nurses need to respect patients from various backgrounds as PWS can affect people of all ethnicities. This syndrome can be found in people of any ethnic background (Zelweger, 1983). Nurses’ must respect the patients’ background and understand that they may not be able to communicate or understand what is being said. Makaton could be used to enhance communication and is a language programme, which is designed to provide a means of communication to individuals who cannot communicate well by speaking (Beukelman. D.R Mirenda). Makaton can also be used with individuals who have cognitive impairments and specific language impairment that have negatively affected the ability to communicate. An interpreter is also a form of communication as they are translating what the other is saying if English is not their first language. By using interpreters (sign language or foreign languages) will help the patient and the nurse understand what the other is saying i.e. explaining what the condition is. This will also leave the patient happy so they do not feel angry and upset. Some individuals may have a language barrier or cultural beliefs which can go against some forms of treatment. The syndrome is lifelong and unfortunately has no cure, but with the support and advice the nurse will have given the patient and the family, the patient will be happy and content (FPWR, 2010). This essay has included the role of the nurse for this condition and how it can be managed. Overall, the main point is to promote a healthy way of what the individual eats and how it can be managed. Keeping such foods out of sight and having a positive family, helping the individual through the tough times can promote a healthy way of living for the individual. Critical Mission of Homeland Security The critical missions of Department of Homeland Security are to prevent, protect, respond and recover. Through utilization of all available technology and resources the primary mission is to prevent an aggressive act. If they are unable to prevent the incident the department will shift focus to protection. Protect as many citizens as possible from any imminent incident. If the incident could not be prevented the Homeland Security officials will respond to whatever the results are of the incident. This response could be in the way of coordinating with other agencies to provide the care and safety needed by the population in the affected area. The final mission is to recover from the incident. Although each mission is defined as a single mission in practice they are often interlinked in execution. While protecting citizens from an incident the agency at the same time may be proactively responding to the results of the incident. The recovery mission is over lapped in many areas by the response mission. Four Foundations of Homeland Security The National Strategy for Homeland Security has defined four foundations of homeland security. These foundations are; law, science and technology, information sharing and systems, and international cooperation. Each of these play a key part in providing for our national security. Identification and enforcement of the law. We have to know exactly what we are enforcing. Use of science and technology in each of the four critical mission areas. Information sharing was something various government agencies were not very effective at prior to 9/11. Since then Homeland Security has been working on removing many of the communication silos that were prevalent in the past. By open communication with several agencies overall effectiveness is improved. International cooperation is the final of the four foundations that has to be built upon. With terrorist moving from nation to nation and planning many of the attacks from foreign soil we have to work with all nations. International borders have to be permeable in today’s war on terror. Virginia Office of Commonwealth Preparedness The Commonwealth of Virginia has their own Department of Homeland Security. In Virginia it is called The Office of Commonwealth Preparedness. This office mirror many of the processes and objectives of the United States Department of Homeland Security. The Office of Commonwealth Preparedness works to make sure that all of the residents and businesses of the Commonwealth are safe, secure and prepared. Criminal Law and Civil Law Criminal Law and Civil Law are very different types of proceedings. Civil Law is usually a case between tow individuals or business entities. Criminal Law is a case between a person and a government. Civil Law In Civil Law a person is involved in litigation with another person or business. These are usually divorce cases, law suits due to injury at a place of business, or bankruptcy cases. Civil law never involves incarceration of either party because of the civil proceedings. There are some Civil cases that may become criminal cases. Stalking a spouse you are involved in a civil divorce proceeding with will lead to criminal charges being filed against the stalker. Civil law will never result in the incarceration of the guilty party. Fines in a Civil Law case are often paid to the other party in the proceeding. Criminal Law In Criminal Law there is a crime committed against society, or that endangers society by the action. Murder is a Criminal Law case. Although there may be only one victim the perpetrator of the criminal act may do so again. Therefore, society must be protected from this person. Driving while intoxicated is also a criminal law case. Even though no member of society may have been harmed by the act of driving under the influence. There was a danger posed to society by the act of driving under the influence. Criminal law cases may result in incarceration or probation of the guilty party. Fines judged in a Criminal Law case are paid to the governing body prosecuting the case. Incarceration for Misdemeanor Crimes The main difference in the two types of crimes misdemeanor and felony is the amount of possible incarceration if convicted. A felony is a more serious crime and if convicted the accused could be sentenced to incarceration for a term greater than one year or death. Misdemeanor crimes are less serious offences and if convicted will result in incarceration of less than a year if at all. I do not think those convicted of a misdemeanor should be incarcerated unless the person demonstrated no willingness to be rehabilitated through other means. Running a red light is a misdemeanor. Incarcerating this person for a period of time will serve no purpose in rehabilitation. The only outcome in this incarceration would be increase cost to the taxpayer of the community and increased crowding of jails. This accused will probably learn their lesson by paying a fine and undergoing a traffic safety class. The penalty of the crime will drive a lesson home to the accused. The taking a class will increase the general safety of the public. The current volume of cases on our court systems and the high cost of incarceration forces us to take a more lenient approach for misdemeanor crimes. Many in society would rather a person creating a danger to the public be off the streets rather than someone convicted of running a traffic light or petty theft. The penalty should be designed to make restitution and lead to rehabilitation of the accused. When we balance the crime with the penalty then we can have a better society. Emergency Response to Terrorism Past Acts of Terrorism On September 11, 2001, America experienced an attack. Early in the morning at around 8:45 am a flight carrying 20, 000 gallons of jet fuel made its way to the northern side of the world trade centre tower hitting the 80th floor. This lead to the death of hundred plus individuals plus other were trapped on the higher floors of the world trade centre located in New York. Some few minutes later the same world trade centre building was hit with another plane which hit the 60th floor, and this led to a massive explosion that alarmed the whole state that indeed America was under attack. Immediately during the attack, the media managed to make a live broadcast of the attack so as to inform the public of the occurrence. The planners of the attack of the September 11 did it perfectly such that they ensured that there was no leak of the attack to the public. The attacker did not make use of any electronic form of communication, and thus this radio silence helped them execute their plan effectively. During the attack, the federal government experienced various communication breakdown, and this affected how the disaster was handled. An official from the 9/1 1gave a statement and said that the federal government experienced various communication breakdowns during the attack (Asaeda, 2005). The report that was given showed that the systems of communication that were put in place had failed. During the attack, those who were operating the planes tried to reach the Federal Aviation Administration via teleconferencing, but this proved difficult. The FAA did not join the conference for almost 15 minutes and even after they had joined they did not take an immediate move. The FAA personals involved in the conferencing did not have the authority to deal with the situation as the senior officials were absent. This delay in the relaying of information between the operators and the FAA contributed a lot to the attack being successful. If the official responded in advance, then an immediate solution would have been adopted to deal with the situation. Immediately the attack took place it only took a few minutes, and the firefighters were on the ground. The attack led to several individuals being hurt and may others losing their lives. The hospitals in New Jersey, New York and the neighbouring areas prepared themselves for the attack. The hospital beds were secured, and the elective surgeries that were taking place were put to a halt to make room for the victims of the attack. St Vincent who was the closest trauma centre approximately 1.6 km from the world trade centre was filled with victims of the occurrence. New York Cornell Hospital dealt with the burn patients as it is the only burn centre in Manhattan. The nurses and the doctors were not allowed to leave the hospitals because of the large number of patients that were being rushed to the hospitals (Kendra Wachtendorf, 2003). The rescue teams including the firefighters and the policemen were in place to ensure that the situation was under control. In total the rescue team mana ged to save the lives of twenty individuals who had been trapped in the building. The first responders encountered several challenges when the WTC attack took place. After the bombing of the world trade centre in 1993 radio repeaters were installed in the tower to ease communication but the firefighters were not aware that the repeaters were functioning. During the attack, there was little communication between the police department and the fire department and thus when the police were notified to evacuate the building after realising that the tower was at a risk of collapsing the firefighters were not informed. The firefighters lacked information about the collapse of the building because they were not watching the news broadcast and also because the communication centre for the police which could have used to inform them had been evacuated due to the threat that the building will collapse. The supplies for various equipment needed for rescue delayed and this thus crippled the rescue situation. The government agencies responded pretty well to the disaster. The various rescue teams and firefighting teams were in place, and they showed their bravery in the scene. They managed to save a lot of lives, and those who were injured were immediately rushed to the hospital. The hospital having been alarmed by the situation had already prepared for the disaster and had their doctors in place to take care of the situation at hand. The rescue teams experienced problems with communication, and thus the crowding of the communication network made it hard for the hospital to determine the patients are coming from the world trade centre attack (Simon Teperman, 2001). Despite the communication breakdown the hospitals still managed to take care of the patients and create more beds for the world trade centre victims. In future, the government should, therefore, ensure that they have communication backup in case of such an occurrence in future. The world trade centre sent an awakening call to the government of the United States concerning their preparation for such disasters. The fact that the tower has been a site of interest for many terrorists because of the many people it accommodates and its location as well. The government should, therefore, put in place security measures that will help curb such situations in future. It involves getting rid of any dangerous objects like bombs or even people that make places unsafe. Cases of decontamination have been very severe as time has gone by mostly encouraged by the fact that there is high rate of technological development. High School In society, schools are one of the institutions that carry the highest number of people. They will have so many people or students who have gone to acquire knowledge and this is almost on a daily basis. Some of the reasons why a school could be attacked include; Many people will die or be affected during the attack due to the high population which is a characteristic of most schools. A terrorist always finds ways of affecting the highest population that he possibly can. The number of deaths that will be experienced in a school is quite high because the school has young children who may not know how to help themselves or how to offer proper first aid to their injured friends due to panic. This means that the number of deaths will be so high. The school has students from almost all walks of life or even at times from different countries and races. Attacking the school will mean that the different families where the children come from will be affected. A large portion of the country and state will be affected in the process. Initial considerations To make a school safe again there are so many things that I will have to look at; The nearness of the school to the city The population of the school is important so that one can prepare and approximate the number of first aid material that will be needed. The Decontamination Once we get to the school; Take all the casualties to the hospital without wasting any time so that we can save as many lifes as possible. This is the very first step to making the school safe by ensuring that the lives of those who have been affected are secured. Once that is done the facility should be cleared so that all people are kept as far away as possible from the school to avoid any more damages. Specialists in the area of bombing are then called in to survey the whole school and check if there are any more bombs. Any that is still on should be disarmed in the most careful way. The school should be then closed till its proved to be safe for any further learning. Football stadium This is a good target for an attack because so many people love football and so he will get the attention he needs from the public. Since different people of different ages and ways of life go to watch football, no one will be able to tell who was responsible for the attack. A person can easily sneak the bomb in and out of the stadium. Initial considerations The architecture of the stadium- Some of the stadiums are quite complex in the way that they were built and this may make it even more difficult to decontaminate. It will require the help of architects especially when it comes to the issue of assessing the place for any bombs that may be hidden. The decontamination Approach scene with caution -While going to the arena, it is important to be careful as you go to the rescue because you can find that there are other bombs in the stadium that are still yet to go off and cause more death. Assess the place and find out where the bombs are located and disarm those that are still on to avoid further damage. This means establishing how the security was bridged all that was done for the bomb to go off It is important to know how many people lost their lives and to also identify who they were. People will come looking for their family members who might have lost their lives and so establishing the identity is important. For accountability purposes, the citizens will also expect to have a comprehensive report on the number of causalities in the bombing. Establish all the resources that will be needed for the cleaning up of any hazardous material left in the stadium. This will rely on the extent to which the bombing affected the place and the size of the stadium. The facility needs to be closed off from any further use till it is completely made safe for use. This will involve clearing of any chemicals or hazardous gases that may have been released. It can only be opened once professionals in this filed certify it as safe for use.
